Printable Aminoglycoside Nomograms

For those wanting extended-interval aminoglycoside nomograms, look no further.  If you’d like a blank nomogram for progress notes or for printing, this simple tool is exactly what you’ve been looking for.

Phenytoin Loading Dose Calculator

Phenytoin (Dilantin) loading doses should be given in patients with subtherapeutic levels or when initiating therapy.  Loading provides rapid attainment of therapeutic levels, but a loading dose is dependent on the patient’s current level and body weight.  This calculator provides an evidence-based pharmacokinetic approach to phenytoin loading.

 

CHADS2 Calculator for Atrial Fibrillation and  CHADS2-VASc Calculator for Atrial Fibrillation

CHADS2 and CHADS2-VASc are scoring systems to guide antithrombotic treatment in atrial fibrillation for the prevention of stroke.  Keep a look out for other related scoring systems, such as HASBLED, to help weigh the risks and benefits of antithrombotic therapy in this patient population.
